Registration Process
Provider registration in the Health Services Directory (HSD) can be completed through this online form only if the practice/business is already listed within Find a Health Service.
This is a two step process involving:
- Entering your individual healthcare provider details and completing the customised privacy settings section within the form. For more information on the different levels of privacy click here.
- Selecting the practice/business that you work within. Providers must select at least one practice that they work within in order to complete the registration process.
